Confirm the following before beginning the application

To continue please confirm that you:

  • have not been convicted of a crime against a child or vulnerable person
  • are able to carry out the roles and responsibilities of a chaperone
  • have completed the online training sessions
  • have a valid Enhanced DBS Children’s Barred List certificate and are registered on the DBS Update Service (within 30 days of your application)
  • consent to Birmingham City Council checking your licence every year
  • are at least 18 years of age

  • your Enhanced DBS certificate number - this has 12 digits and starts with 00
  • your Enhanced DBS certificate is registered on the Update Service
  • certificates to show you have completed both chaperone training courses
  • a recent passport-style photo that shows your entire head and shoulders
  • the name and email addresses of 2 referees. They cannot be members of your family and must have known you for at least 2 years

  • be clear in contrast to the background

  • 'None recorded' for DBS Adults' Barred List information
